PLEASE READ THIS NOTICE CAREFULLY BEFORE SUBMITTING YOUR APPLICATION.
1. Application Does Not Guarantee Approval
Submission of this application to the Velaris Verified Operator Program does not constitute an offer of participation, an approval, or a guarantee of any compensation. All applications are subject to manual review by Velaris's onboarding and compliance team. Velaris reserves the right to approve or decline any application at its sole discretion.
You will be notified of your application status within 3–5 business days of submission. In some cases, additional documentation or information may be requested before a final determination is made.
2. Purpose of Information Collection
The personal information you provide in this application — including your name, contact information, Social Security Number, government-issued identification, and financial history disclosures — is collected solely for the following purposes:
- Identity verification and KYC compliance
- Background check processing
- Application review and eligibility determination
- LLC formation and business banking coordination (for approved applicants)
- Compliance record-keeping as required by applicable law
Your information is not sold to third parties. It is shared only with service providers directly involved in the application review and onboarding process, as described in our Privacy Policy.
3. Background Check Notice
As part of the application review process, Velaris may conduct a consumer background check through a licensed consumer reporting agency. By submitting this application, you acknowledge that a background check may be performed. You have rights under the Fair Credit Reporting Act (FCRA) in connection with any consumer report obtained.
If a background check results in an adverse action on your application, you will receive a pre-adverse action notice as required by the FCRA, including a copy of the consumer report and a summary of your rights.
4. No Employment Relationship Created
Submission or approval of this application does not create an employment relationship between you and Velaris LLC. If approved, you will participate in the Velaris Verified Operator Program as an independent contractor under a separate Participation Agreement, which you will receive and review before any commitment is made.
